Notice of Claim

Instructions for Online Use

Please print three (3) copies of the Notice of Claim against the City of Albany to be submitted by you in connection with your claim.  The Notice of Claim must be either mailed or delivered in the method described below within ninety (90) days of the date of the incident.  Complete the Notice of Claim form with a brief description as to what happened.  All Notice of Claims against the City of Albany must be in writing, and they must be sworn to by or on behalf of the claimant.

Please include as much of the information outlined below, as it is relevant to your claim. The more complete your Notice of Claim, the easier it will be to process.
  • The full name and mailing address of the claimant, as well as home and work telephone numbers.
  • The name and post-office address of your attorney, if any.
  • The nature of the claim.
  • The time when, the place where, and the manner in which the claim arose.
  • If the claim involves a vehicle, please make sure that you have included the make, model and license plate number of the vehicle, as well as whether the vehicle is owned by the person asserting the claim.
  • For wrongful tow claims, the bill or receipt from the towing company should be included.
  • Proof of damages is not necessary in a Notice of Claim for personal injury.   Just briefly describe the injury or injuries sustained.
  • Every Notice of Claim should provide the precise location where the incident giving rise to the claim occurred. 

Upon completion of the Notice of Claim, two copies must be notarized.  This must be done before submission to this office. Notice of Claims that are not notarized, will be rejected

You may serve your Notice of Claim by registered or certified mail, return receipt requested, upon:

In the alternative, your Notice of Claim may be hand delivered to one of the Assistant Corporation Counsels at the Office of the Corporation Counsel, City Hall, Albany, New York, weekdays from 9:00 a.m. to 5:00 p.m.
