    1. This year, the City of Albany will be presenting its 4th Annual Henry Johnson Award for Distinguished Community Service. If you know an Albany resident who has made meaningful contributions to our community in arts and history, social justice, education or volunteerism, please complete the following form. All nominations for the Henry Johnson Award must be submitted by 5 pm on Friday, August 28. The awardee will be notified by email or by phone before a ceremony to be held in the fall at a date to be determined.
    2. Rules
      All nominations will be reviewed by the Henry Johnson Award Committee, made up of members from the City's all-volunteer Cultural Heritage and Tourism Partnership and the Henry Johnson Day Working Group.
    3. Individuals may not nominate themselves, nor may they be nominated by members of the Award Committee. They may be nominated by family members, friends, neighbors, colleagues/coworkers and employees.
    4. The winner will be selected at the discretion of the Award Committee, based on the strength of the nomination, including the length of service to the community and impact of work as described by the nominator.
